Please visit our medical records kiosk located in Buck Pavilion lobby, adjacent to the main reception desk or use one of the options below to request your records electronically or by mail. Need to request your medical records?
We offer two options; electronic delivery or mail delivery. Choose the option that is most convenient for you.
Electronic delivery
Complete a request from your phone, tablet or computer and your records are delivered right to you - no need to pick them up. You'll get an email from Datavant with instructions for retrieving your records. To keep them secure, you'll need a PIN that is sent in a separate email.Submit a request online
Mail Delivery
Complete and sign the authorization for the release of information form and submit by fax (203) 749-9000 or email

If you identify that information in your medical record is incorrect, please submit a completed form to amend health information. Include your current address and phone number.

Requesting records for a deceased patient
If copies of your records are going directly to a physician or hospital, there is no charge. If copies are being sent directly to you, there is a fee of 12 cents per page. An additional form is required to obtain records for a deceased patient. Please find additional forms below:
