Our unique partnership is located right inside your primary care office and allows for the identification of mental health concerns and major mental health issues that may need referrals for additional treatment.-
What is a Behavioral Health Consultant?
Our team consists of experienced, Licensed Clinical Social Workers (LCSW) who provide medical and behavioral health-focused care for problems resulting from medical illnesses, mental health related issues or substance use. Our team utilizes a special set of skills to help you and your family more successfully manage illnesses through the continuum of care.

Meet the Rhinebeck team
Cheranique Jones, LCSW
Cheranique Jones has been a medical social worker for over 15 years. She obtained her master’s in social work from Fordham University in 2007 and is licensed in New York and Connecticut. Cheranique has extensive experience working with a diverse population adjusting to chronic illnesses. When working with clients, Cheranique provides a holistic, compassionate and person-centered approach to therapy and uses a special set of skills by blending modalities in brief talk therapy sessions that include somatic, mindfulness, motivational interviewing and solution-focused treatment. She received her certification as a Shame-Informed Treatment Specialist, focusing on eating disorders, addictive eating and body shame and is working towards obtaining her CASAC credentials. She is very excited to be the LCSW providing direct clinical services to the Bariatric/Metabolic Weight loss program at Nuvance Health -Northern Dutchess Hospital. This collaborative approach to providing care will help patients improve their insight and empower them to make changes that are beneficial to their emotional and physical wellbeing.
During Cheranique’s free-time she enjoys listening to music, movies, traveling and spending time with family and friends.
